This report documents the design and administration of a survey of Northern California commuters.  The survey is the data collection instrument for a study of multitasking and its impact on commute mode choice and value of travel time.  It consists of nine parts, with the paper version comprising 14 pages and the online version including all the contents of the paper version plus a few more questions.  A variety of survey distribution methods were used, with the goal of obtaining data for sizable numbers of commuters by each of the major means of transportation.  The report describes the content of the survey and its rationale, as well as the sampling plan and its execution.
